Egzoszkielety radiacyjneRadiation-protective exoskeletons
Osłona, której ciężaru nie niesie kręgosłup. Osłony o ekwiwalencie ołowiu wiszą na zewnętrznym rusztowaniu egzoszkieletu, a wysoka przyłbica osłania to, czego fartuch z kołnierzem chroni słabo albo wcale: mózg, oczy i tarczycę. Shielding whose weight the spine does not carry. Lead-equivalent shields hang on the exoskeleton's external frame, and a tall visor covers what an apron with a collar protects poorly or not at all: the brain, the eyes and the thyroid. We implemented the system at USK No. 2 PUM as the first center in Poland and the second in Europe, and its efficacy is being verified by PROTECT-EVAR, the world's first randomized trial of exoskeletons in endovascular aortic surgery.
Fartuch chroni tułów. Egzoszkielet chroni operatora. An apron protects the torso. An exoskeleton protects the operator.
During endovascular procedures the operator stands at the table for hours within reach of scattered radiation from the patient. A standard lead apron with a thyroid collar shields the torso and, to a degree, the thyroid, but not the head: the brain and the eyes stay exposed, and the spine carries the apron's weight throughout the procedure. A radiation-protective exoskeleton solves both problems at once. Lead-equivalent shields hang on an external frame that carries their weight to the floor, bypassing the operator's spine, and a tall visor covers the face, the eyes and the neck more completely than a collar.
Wdrożenie rozszerzamy w polsko-niemieckim ekosystemie szkoleniowo-badawczym SKILLAB (Interreg, USK-2 PUM i Universitätsmedizin Greifswald), a dane poziomu I dostarczy badanie PROTECT-EVAR. The effect is measurable: compared with an apron and collar, the dose to the operator's brain, eyes and thyroid drops by more than 90%. The rollout is being extended within the Polish-German SKILLAB training-and-research ecosystem (Interreg, USK-2 PUM and Universitätsmedizin Greifswald), and level-I data will come from the PROTECT-EVAR trial.
Ubieranie egzoszkieletuPutting the exoskeleton on
Egzoszkielet czeka zadokowany w stojaku. Operator wchodzi w niego tyłem, zapina uprząż, opuszcza przyłbicę i odchodzi od stojaka gotowy do zabiegu. Na nagraniu cała procedura trwa niespełna pół minuty. The exoskeleton waits docked in its stand. The operator backs into it, fastens the harness, lowers the visor and walks away from the stand ready for the procedure. In the recording the whole routine takes under half a minute.
Between procedures the system returns to the stand: the weight of the shields never rests on the operator, even when not working at the table.

Opuszczona przed twarz przyłbica osłania mózg, oczy i tarczycę: mózgu i oczu fartuch nie chroni wcale, tarczycy tylko kołnierzem.Lowered in front of the face, the visor covers the brain, the eyes and the thyroid: an apron leaves the brain and the eyes unprotected, and the thyroid only behind a collar.
Operator pracuje przy stole jak zwykle. W badaniu PROTECT-EVAR dawka jest mierzona w czasie rzeczywistym, a obciążenie mięśniowo-szkieletowe porównywane z fartuchem ołowianym. The operator works at the table as usual. In the PROTECT-EVAR trial the dose is measured in real time and the musculoskeletal strain is compared with a lead apron.

PROTECT-EVAR
Jestem pomysłodawcą i kierownikiem klinicznym badania. A randomized crossover trial of radiation-protective exoskeletons versus lead aprons, the first of its kind worldwide. Two parallel measurements: real-time dosimetry and the operator's musculoskeletal strain. The goal is level-I data for ESVS, ESC and CIRSE guidance. I am the originator and clinical lead of the trial.
